2 Quick Apply Full-time 1 hour ago Full Job Description CSS is a private not-for-profit social/human services agency that initiates, provides and promotes services for people with intellectual/developmental disabilities and their families, within their communities, in order to strengthen their independence, self-esteem and ability to participate in and contribute to community life.
Service Coordinators may be assigned to any of Community Support Services' programs,including but not limited to:
Residential (24 Hour CILA), Independent Living (Intermittent CILA), Day Program (Community360), Home Based Services,and/or Case Management/Respite.
? Duties &
Responsibilities:
Specific 1.
Advocate,explain and protect the individual's rights and services.
2.
Responsible for all service functions including assessment, planning, coordination anddelivery.
3.
Convene the Support Team, initiate and modify the individual services plan as part of the interdisciplinary process.
4.
Assure the participation of the individual, team members, the guardian, the family and/or friends, and the necessary professional in the interdisciplinary team process, unless the individual is their own guardian and does not desire the involvement of the family or the family refuses to participate.
5.
Responsible for providing or helping individuals to access the services specified in theirplans.
6.
Monitor and document program individual's progress in relation to the services plan.
7.
Assure that services specified in the Service Plan are being implemented.
8.
Identify and address gaps in service provision.
9.
Serveas a liaison, when necessary, between CSS and other facilities, such asstate-operated facilities, convalescent care facilities, community hospitals or rehabilitation facilities.
10.
Develop natural communitysupports, fostering relationships with non-paid persons in the community, e.
g.
,neighbors, volunteers, and landlords.
11.
Work collaboratively with other agencies to promote the coordination of a network of services.
12.
May supervise, recruit, hire, discipline, etc.
Direct Support Professionals.
13.
May assist individuals and families with the hiring and supervising of Direct Support Professionals, if appropriate.
14.
Provide personal support and assistance in gaining access to vocational training, educational services, legal services, employment opportunities, and leisure, recreation, religious,and social activities.
15.
Provide assistance in obtaining health and dental services, mental health treatment and rehabilitation services (including physical therapy and occupational therapy),and substance abuse services.
16.
Access medication information including observing and reporting effects and side effects ofprescribed medications; provide medication administration if/when needed and appropriate.
17.
Provide support and problem-solving assistance on an ongoing basis and at times of crisis.
18.
Provide assistance inmoney management, including representative payee-ship, and applying for entitlements, including LINK, if appropriate.
19.
Provide informational,educational, and advocacy services to program individuals and family members.
Qualifications:
Required Education:
Bachelor's degree in Human Services or other related degree, approved by Department of Human Services.
Required
Experience:
Minimum of one year's experience working with people with developmental disabilities.
